New Patient Information

Welcome to Pekoe! We are looking forward to working with you. Get ready for your first appointment with the checklist below!

Let’s Get Started

Please fill out our New Patient Intake Form. This provides us with emergency contact information, consent to treatment, and informs you of our policies. Any health history you can provide will help us to better serve you.

Completing the intake forms will automatically create an account you can use to book appointments and access patient resources. You may also make an account through the link under Quick Links.

If you are interested in using insurance benefits for your acupuncture appointment, please submit your insurance information under your preferred practitioner below.

Book an appointment, explore our services, get to know our providers, and find a good time to come in!

If you need to cancel or reschedule your appointment, please let us know at least 24 hours in advance of your appointment time to avoid a late cancellation fee.

If you have any questions or would like to request a translator, accommodations for mobility, or other considerations, please contact us! We are happy to work with you, and ask you communicate with as much advance notice as possible to allow time for any arrangements.

Insurance Benefits

We can bill acupuncture treatments directly to insurance carriers. Your benefits will depend on your specific plan. Please see the list of in-network providers below.

If your preferred practitioner is not in network with your insurance, you can still submit your information to find out any applicable out-of-network benefits, which may differ from your in-network benefits.

We cannot bill insurance until we have verified your benefits (which takes about 3 business days). To avoid paying the out-of-pocket cost at the time of service, please provide us with all your insurance information prior to your first visit.

Please make sure you read and understand our Cancelation Policy:

To ensure fairness for our practitioners and other clients, we ask for 24 hours advance notice if you need to cancel or reschedule your appointment. Canceling or rescheduling within 24 hours of your appointment time, or missing the appointment, will result in a late cancelation fee of half the appointment value - discounts and insurance coverage do not apply. We may make exceptions for emergencies or illness, but encourage you to think ahead and give us as much advance notice as possible. Cancelling via email or phone message is acceptable even outside of business hours. Thanks for your cooperation!
