New Patient Information

Welcome! I am looking forward to working with you. Get ready for your first appointment with the checklist below!

Let’s Get Started

  • Please fill out the New Patient Intake Form on your patient portal. This provides me with emergency contact information, consent to treatment, and informs you of our policies. Any health history you can provide will help me to better serve you.
  • If you are interested in using insurance benefits for your acupuncture appointment, please submit your insurance information below. I use a third-party biller for eligibility and claims submission. 

  • Use the booking link, explore the services, and find a good time to come in!

  • If you need to cancel or reschedule your appointment, please let me know at least 24 hours in advance of your appointment time to avoid a late cancellation fee (50% of the appointment).

  • If you have any questions or would like to request a translator, accommodations for mobility, or other considerations, please contact me! I am happy to work with you, and ask you to communicate with as much advance notice as possible to allow time for any arrangements.

  • This is a home office in a DC rowhouse. There are two sets of three steps to get to the front door, and the treatment room is up one flight of stairs. 
  • Brigid-the-dog and Tabbytha Robert-the-cat will be present. Please let me know if you have any fears or allergies, and I can accommodate to your severity. 
  • You will receive a text reminder. This is a two-way number that can be used for quick communication. Otherwise, please use the patient portal for efficient communication! 

Insurance Benefits

I can bill acupuncture treatments directly to insurance carriers. Your benefits will depend on your specific plan.

If I am not in network with your insurance, you can still submit your information to find out any applicable out-of-network benefits, which may differ from your in-network benefits.

I cannot bill insurance until we have verified your benefits (which takes about 3 business days). To avoid paying the out-of-pocket cost at the time of service, please provide me with all your insurance information prior to your first visit.
