- Please fill out our New Patient Intake Form. This provides us with emergency contact information, consent to treatment, and informs you of our policies. Any health history you can provide will help us to better serve you.

If you are interested in using insurance benefits for your acupuncture appointment, please submit your insurance information under your preferred practitioner below.

If you need to cancel or reschedule your appointment, please let us know at least 24 hours in advance of your appointment time to avoid a late cancellation fee.

If you have any questions or would like to request a translator, accommodations for mobility, or other considerations, please contact us! We are happy to work with you, and ask you communicate with as much advance notice as possible to allow time for any arrangements.

Insurance Benefits
If your preferred practitioner is not in network with your insurance, you can still submit your information to find out any applicable out-of-network benefits, which may differ from your in-network benefits.
We cannot bill insurance until we have verified your benefits (which takes about 3 business days). To avoid paying the out-of-pocket cost at the time of service, please provide us with all your insurance information prior to your first visit.
